The Federal Communications Commission (FCC) Wireless Telecommunications Bureau issues licenses for broadband and similar radio services. Applicants use FCC Form 601 to apply for this type of radio license.

Access the “FCC Form 601″ page on the Federal Communications Commission website to get started. There is a link to that page in the Resources section near the end of this article.

Near the top of the page, you may access the online 601 form if you wish. Even if you do that, it is sometimes better to print out all the documents first, study them, read the instructions, fill them out, and then fill out and submit the online form. Otherwise, you may get confused a lot as you are trying to fill out the online form.

On the main 601 page, you can find links to all the Schedule forms that you may need. As of September 2011, the following Schedules must be filed by certain filers with the Wireless Telecommunications Bureau for radio-service authorization: A, B, D, E, F, G, H, I, J, K, L, and M.

Other forms that you may need to submit the Form 601 are FCC Forms 159, 160, and 606. On the main 601 page, you will see links to those forms, as well.

There is a very large file linked to below with the main 601 form, all schedules, and instructions. This thing is over 100 pages, so you may wish to open it and use it as a guide while separately opening up the main Form 601 and any Schedule forms you need to submit to the Wireless Telecommunications Bureau.
