UTI or Kidney Stones?

Q: I am going to get an ultrasound soon, but i still would like to see what you think. Here are my symptoms:I went to the doctor for a UTI. They gave me medicine, but it has not helped AT ALL.I experience a light sharp pain constantly, but once in a while i will get very sick and my side/flank will hurt like heck. Where my left kidney is, i feel sharp, stabbing pains and it hurts. I have to pee a lot more than usual, and the urge is very strong. There is some microscopic blood in my urine.Just today, i coughed really hard. I felt something shift where my kidney is. It hurt like HELL for a few seconds, then it faded. Q: I had kidney stones a year ago but I still have a constant ache. I assumed it would go away but it didn’t. I went to doctors and he told me there were five possibilities; new kidney stone, infection, kidney trauma, enlarged kidney, or blood in/surrounding my kidney. Through test and x-rays we eliminated kidney stones or an infection. So, my questions are what do the last three options mean(what are they)? How are they treated? What is the possibility of each being my problem, how common are they? Or, what else could it be? Here are my symptoms. Constant aches, within my body not the skin, aches get worse when I am cold, I’ve had it for a year and it has not gotten any better/worse, I have not had any illnesses since(chills, fevers), I had lithotripsy to break up the original kidney stone, I have a scar from it, I had a shunt. A: By “shunt,” I assume you mean stent, which is a narrow plastic tube that keeps the ureter open after procedures. If you still have the stent in place, then some symptoms are normal, including feeling the need to urinate frequently and feeling an ache in your kidney, particularly when you feel the need to urinate. That last effect is due to pressure from the bladder being transmitted up the stent to the kidney, which feels a lot like a kidney stone. If, however, the stent was already removed and infection has already been ruled out, then an important issue to rule out is obstruction of the ureter. This can be caused by scar tissue from the stone or by a congenital defect. Such congenital obstruction, most commonly at the junction of the urinary collecting part of the kidney and the ureter (ureteropelvic junction or UPJ). This situation can be evaluated by a contrasted set of X-Rays such as an IVP or by a functional drainage study of the kidney such Mag3 Lasix Renogram. If you have obstruction, it should be treated, as it puts you at increased risk of forming more kidney stones.Be sure the stent was removed! If a foreign body is left within the urinary tract for more than a few months, it can become encrusted with crystals and cause all sorts of problems.

Q: What are the symptoms?. It is a silent fear I have as I have had stones twice before and recently keep getting extreme back pain similar to when I had the calculus (stones). But this is more persistent and I keep thinking that my right kidney is a serious problem. A: Its not very common but it’s not exactly rare to get it either. You are more like to have kidney disease then kidney cancer. You should get your doctor to order blood and urine tests and also a renal ultrasound at the very least.
