How to Obtain FCC Forms and Applications

The Federal Communications Commission regulates the public airwaves in the United States. Among other things, it has forms to issue station and operator licenses and take complaints from the public over decency and advertising rule violations.

The main place to find FCC forms and applications online is on the “Forms” page linked to below. On that page, there are dozens of FCC forms that can be downloaded and printed from a home or office computer. Some forms must be filed electronically. There is a link on the page if that applies to the form or application that you are trying to file.

Another way to get a form is to call the FCC and place an order. This may carry a fee. You will have to ask the customer service rep about that when you call. The phone number to order FCC forms and applications is 800-418-3676. This is the number for the Federal Communications Commission Forms Distribution Center.

To get assistance in filling out a form or application, you can call the FCC Consumer and Governmental Affairs Bureau. The number is 888-CALL-FCC.

Separately, you may also file a complaint online if you feel that you are being harassed by illegal junk faxes or telephone calls that violate telemarketing rules. Form 1088 is a consumer complaint form and can be filed online using the link below.
