It is caused by the bites of infected mosquitoes. Some cases develop much milder symptoms, which can, when no rash is present, be misdiagnosed as a flu or other viral infection. Thus, travelers from tropical areas may inadvertently pass on dengue in their home countries, having not being properly diagnosed at the height of their illness. Patients with dengue can only pass on the infection through mosquitoes or blood products while they are still febrile. The classic dengue fever lasts about six to seven days, with a smaller peak of fever at the trailing end of the fever.
Dengue virus affects the Reticuloendothelial system (RE System) and therefore we see petichae, platelet consumption coagulopathy and bleeding disorders. This can be severe enough so as to warrant infusion of fresh platelets or in the absence of the blood fractions, whole blood may have to be used in order to raise and maintain the levels of platelets in the body. The gross effect is directly seen over the spleen and the smaller arterioles and venioles. The other manifestation of the disease such as fever, aches, prostration, are because of the activation of cytokinins and the release of the mediators of the inflammation.
