Weird “Slow” Heart Rhythm- Why?
Q: OK, well here’s what happened. I’m 15 years old, 110 pounds and 5’7″ (just background). I am in decent shape and run miles pretty well (average is about 6:08). Last year I was having some odd heart palpitations after running sprints in track, in which my heart would seem to skip, and I’d feel very weak for a while sometimes. Cardiologist checked it out and said everything was just fine, so I assumed I was just not in shape enough to do them without stuff like that…Well today in P.E. our coach had us run a very short distance (perhaps 100m). I took off fast, but noticed something felt weird in my “heart” area. It was as if nothing was there at all. When I stopped and put my finger where my heart was it was beating very slowly, probably 2/3 of my resting heart rate (which I believe is around 68BPM). I was getting afraid because obviously if I sprinted my heart rate should go up, not down… I started to get slightly dizzy (it’s hard to remember too much because I started to panic) and after about 45 seconds of this slow heart beat I started to tell my coach. A few seconds later my heart rate instantly jumped back up to the much much quicker rate I’d expect afterwards. After class when my coach told me to go to the nurse, she said my blood pressure was perfect and my pulse something like 75BPM which was fine. So… um… anyone know what to make of this? Was it some freak heart rhythm I shouldn’t worry about? I mean it did last quite a bit. My heart seems to have been beating more or less normally (I did note that it was beating “different” when I had my hand on it though I don’t know what that was). Thanks for your help. Ah, thanks for the input. I did the stress test and I did very well according to the doctor. I didn’t have those weird heart rhythms during it though; I think it’s only when I suddenly sprint or slow down that my heart has issues. Never wore the monitor though. At any rate nothing was ever found, so I assumed it was all in my head. This time however with my hand on my heart I know for sure that my rhythm was off and that I wasn’t just imagining the sensation.Oh yeah, and 68 is my RESTING heart rate. It is a usually a lot higher when I run. I mean that after the run my heart was like 50 BPM.
A: You need to have a stress test where you are hooked up to ecg leads and on a treadmill.. A heart rate of 68 is not slow, but it should be higher if you are running. What did your heart increase to you don’t state. Your Dr.should order a holter monitor for you regardless so there is documentation of your heart rhythm. I don’t think you should be running until your Dr. gets to the bottom of this, take care, Donna I just went back and read your previous questions, what does your Dr.say about your short PR interval, and no it’s not always normal as the responder to your question has written, you need those tests done and if anything is discovered you should be referred to a cardiac electrophysiologist for his interpretation of things, and I don’t think you should be running until you get the all clear from a cardiologist, take care Donna
Should you trust your gut instincts over your physicians diagnosis?
Q: I am 24 years old and as an infant I was diagnosed with a hole in my heart. My doctor told my parents that surgery wasn’t necessary and that eventually the hole in my heart would close on its own which it eventually did. However I have had a slight murmur ever since. It is something that has never caused me any problems. I am very athletic, don’t drink or smoke and eat healthy and at my last full physical was told that my health was good.Things changed about two weeks ago. I was lying down and suddenly I felt this sharp stabbing between my breast that lasted for a few seconds before disappearing. Afterwards, I had this strange heavy feeling in my chest that last for at least ten minutes. It was scary but I thought maybe I had gas or something and shrugged it off as nothing to worry about. Then two days later it happened again, only this time once the heavy feeling subsided, I had a feeling I could only describe as strange followed by this very hard forceful heartbeat. At this point I was scared.I made an appointment with a cardiologist when my scary experience turned terrifying. On the morning of my appointment all the same symptoms described above returned only when it was all said and done my heart was beating so fast I thought I was having a heart attack. My boyfriend rushed me to the ER where the cardiologist I made my appointment with worked. After several hours and diagnositc tests he told me that my heart murmur was just aggravated and that sometimes it happens due to overexcersion of excersize and other physical activities. He told me to rest and that I would be fine. However I don’t feel fine. In fact I feel awful and I feel as if my doctor missed something. Although I haven’t had any “incidents” since that day I feel tired all the time and my heart is beating slow yet forceful. A friend of mine who is a medical student frightened me the other day when he said it sounded like it was taking all the strength my heart had to keep beating. I’m scared about it and unsure what to do. I mean I want to trust my doctor because he is a professional and he knows what he’s doing, however I just can’t shake the feeling that something is very wrong. Should I seek a second opinion or is it my fear over the original incident causing me to panic over something that I shouldn’t be?
A: I would trust your Physician’s diagnosis. The information he obtained would have come from different machines run by different technicians, who would have all been checking for the same types of issues. Medical care is not the result of one Doctor looking at your symptoms and guessing. It is a collaborative effort of all healthcare practitioners and corresponding diagnostic tests. The circulatory system is 1 of the most straight forward systems in the body, and operate very mechanically. An EKG or ECG (electrocardiogram) will show the electrical impulses around your heart. The paths that these impulses take will indicate if other conditions (not related to the electrical path directly) exist as well. Another valued diagnostic test is an echo or echocardiogram. This is an ultrasound of your heart, and will show the heart visually in real time, as well as obtain velocity values of the blood flow throughout the heart. It will determine the cause and quantify the severity of your murmur. There are other tests you could have as well, but these two would be the most relevant given your circumstances. Was the shunt (hole) in your heart an ASD (atrial septal defect) or a VSD (ventricular septal defect)? Both of these would be seen on an echo. It is more likely that your symptoms stem from anxiety from your perceived condition after it was aggravated as your doctor said. If you continue having anxiety (which often presents as cardiac symptoms, among other things) you may speak to your doctor about medication to treat the anxiety (A benzodiazepine for example) or practice breathing exercises, yoga, meditation, etc.) Hope this helps. Q: No one ever said going to a public school was fun. Going to a big public school in a metropolis, sharing a classroom with thirty five other juveniles is the opposite of what I like to call, fun or even educational. It would be ok, if the students were nice, and liked to come to school, but here in West Mesa, Albuquerque, about 95% of the students are forced to mark their presence in school every day by their parole officer. The other 5%, which includes me, sees schools as just of way of getting out of their home, away from their parents. The hot sun stretched its ray across the cloudless sky of the summer’s afternoon. It was the last period of the day, and everyone could not wait for the ancient bell to ring. Mrs. Maribel, the white haired and wrinkled history teacher, decided not to teach today. Rumors were that her husband had an affair with the art teacher, who was very youthful and had a band of school boys running after her. So Mrs. Maribel had been unusually gloomy. She arrived in class a few minutes late, with red eyes, placed her books in the old wooden table and said that we should catch up on our work, which meant that we could do whatever we wanted. I sighed, and opened my fake coach bag, and pulled the Elle magazine, which had just arrived in the mail this morning, and my pink IPod nano. I turned the volume on high and immersed myself in the new fashion trends and some gossip. She dropped her arms next to her looked around to see everyone’s surprised stare.“I’m sorry.” She said miserably with a heavy accent.She raised her right hand that was holding the silver gun and brought it to the side of her head. A last tear rolled down her cheeks. “No, STOP!” I yelled She closed her eyes and slowly and pulled the trigger.
