Help with diagnosis of abdominal pain?
Q: I am a fifteen-year-old female grade nine student. I don’t eat regularly, or eat healthily most of the time. Usually I skip lunch or breakfast, and sometimes supper. I’m 5’3 ½ and I’m about the right weight for my height and age. I have trouble falling asleep, and am usually up until midnight-2am. I get up at 7 am. So I usually get between 5-7 hours of sleep a night. I don’t exercise regularly, either.Since about February (so for like 2 months), I’ve been feeling near-constant pain in my lower and left abdomen. I often get chills and cramps so bad that I have to double over. I also have to double over to muffle the gurgling sound coming from my stomach.It’s not a gurgling from being hungry. It’s quite different, and it sounds like it comes from my left side. Also, I drink a substance to curve my appetite so my stomach won’t growl from hunger in class – therefore, it’s not growling from being hungry, as the drink makes me feel full.My symptoms include: Dull pain near the navel, loss of appetite, nausea soon after abdominal pain begins, abdominal swelling, bloating, inability to pass gas – or too frequent gas, dull/sharp pain near my left/lower abdomen, severe cramps, constipation, weight loss, pain in the chest, behind breastbone (once or twice), coughing, hoarseness, indigestion, (I get secondhand smoke), weakness, pain/tenderness in the lower left side of the abdomen, chills, fever, stomach cramps, tiredness.I get dull, gnawing stomach pain that comes and goes. The pain is often worse when my stomach is empty, and sometimes goes away after eating. I don’t think I digest properly, and sometimes feel nauseated. Often, I feel like I have to pas gas, but sometimes I can’t, or I have to hold it in.Also – “Abdominal pain in IBS is often characterized as being intermittent or constant, tender upon palpation and can be relieved by passing gas or having a bowel movement. It has also been described along a range from mild to severe. The pain can become so severe that it can affect the sufferer’s quality of life and cause the person to miss school, work, and other social events.” My pain on the left/lower side ranges from mild to severe, and sometimes can be relieved by passing gas, having a bowel movement, or eating. Recently, it’s been getting worse and worse and I keep missing school, and I fear that I may fail or something. But it’s not my fault – it’s so distracting that I can’t think or concentrate, and it’s hard to try and work when you can feel and hear your abdominal organs messing around.My symptoms point to some of the following:-Appendicitis-Esophageal Cancer (I don’t think I have cancer)-IBS; Irritable Bowel Syndrome-Diverticulitis-A stomach ulcer (peptic ulcer)-Gastritis (inflammation of the stomach lining)-Small or large bowel obstruction-Ulcerative colitis (with pain in the left side)-Dysentery-A mass-A hernia (both my mom and her brother have them)-An obstruction-Indigestion-ConstipationI can’t go see a doctor right now because my mom has to fill out some forms. I would go to the walk-in clinic, but I’m not sure how much help they’ll be. What would I tell the doctors anyway? I couldn’t just give them the list of my symptoms, it would probably be shrugged off as indigestion or constipation. And I don’t think this can be shrugged off as constipation or indigestion if it happens so frequently unless there’s something wrong with my diet/body/lifestyle. But then – how would my diet cause fevers, chills, and so much pain?The pain comes and goes so I can’t ever say when it will occur. A: See your health care provider as soon as possible. Abdominal pain can be a sign of a serious condition. Fortunately, minor causes are much more frequent. Location of the pain can help in suggesting the cause.Appendix pain usually occurs in the right lower quarter of the abdomen.Diverticulitis usually hurts in the left lower quarter of the abdomen.Kidney pain, the back.Gallbladder, the right upper quarter.Stomach, the upper abdomen.Bladder or female organs, the lower areas.Exceptions to these rules do occur.Pain from hollow organs-such as the bowel or gallbladder-tends to be intermittent and resembles gas pain or colic. Pain from solid organs-kidneys, spleen, liver tends to be more constant. Stomach ulcers tend to create burning pain in the upper abdomen which usually gets better after a meal or a dose of antacid. There are exceptions to these rules as well.If the pain is very severe or if bleeding from the bowel occurs, see a doctor. Q: LONG story, please do not comment about how it was too long, I warned you in the beginning. My husband and I planned this pregnancy, I knew everything about my cycle, I knew I ovulated on the 10th of Feb, and I tested positive on a First Response test on the 19th of Feb. In total I took 5 pregnancy tests all positives and even went and had my pregnancy confirmed on the 23rd.Well, around the 22nd, I started having some abdominal pain and craming and midback pain, I’d had the pain before when I had a UTI so didn’t think anything of it. But on the 24th while at work I was unable to stand up straight from the pain. I was rushed to the ER where I told them I was only 4 weeks pregnant (aka 2 weeks from conception) and I told them about my medical history of PCOS and UTI’s. Well after 2 ultrasounds the ultrasound tech told me that you can’t see anything usually until after 3 weeks post conception, which I understood. Anyways, fast forward 6 hours later.A nurse comes in my room and tells me she’s going to give me morphine for the pain, at this I immediately say “No, I’m sure that isn’t good for the baby.” And at that she looked up at me and then to my husband and said, “So no one has been in to talk to you yet then?” And she excused herself only to come back and give me the injection telling me it would be alright for my “pregnancy”… after 20 minutes of morphine my dr who I had only seen once before came in the room and told me it was an ectopic pregnancy and they had to give me two injections to get rid of it. I broke down. I knew this was my baby, it was our first try and I had all the cards stacked against me with PCOS and thyroid disease. So… I accepted it and they gave me the two injections and told me that they would watch me for 30 min to see how I reacted. I didn’t know what that meant but I trusted them.Last Friday I went to my own OB/GYN and had my hcg levels retested and an ultrasound. My hcg levels had doubled since they were tested in the ER on Wednesday, and on the ultrasound they still saw nothing. Another thing, all my pain had gone away, except for emotional pain of course.Well, Saturday I got really sick, I honestly thought I was dying. Worst cough of my life, I pulled muscles in my abdomen from it, high fever, unable to breathe easily, so I started researching the injections they gave me, Methotrexate. I was so surprised they NEVER explained what it was, what it did, that it was used in chemo, that it could lower my blood count which it did, I was shocked they had let that slip. Monday I went back to the Dr again for more HCG tests, at that appointment I was told my levels had doubled again and are now almost at 1000, only being 2.5 weeks pregnant, which was right on track for a regular pregnancy. My dr told me she hadn’t seen many ectopic pregnancies get out of the 400hcg range, nor had she seen any women’s levels continue to double after the MTX injections. She also continued to tell me, she had looked over my chart from the night at the ER and she had not seen anything besides abdominal pain to support their diagnosis. At this, I asked her if it were possible they were wrong and she said, it is very possible.I got my medical records within the hour after that conversation from the ER I had gone to, and went over my file myself. I looked at my own sonos and saw nothing but cysts on my ovaries which were NORMAL for me, i had told them about the PCOS already, and I thought “there is NO way they could of done that..” also noted was there was a fairly large amount of fluid in my abdomen. I’ve had cysts rupture before that were the size of oranges, and that was what my own OB told me she believed happened that day. So now, I’m wondering what to do? I find out tomorrow for sure if it is an intrauterine pregnancy, and if it is we now have to live with the fact that due to the Methrotrexate they gave me, my baby will likely be born with severe birth defects or mental disabilities. I’m flat out angry about this whole situation, I trusted the ER doctors to tell me the truth, to tell me they weren’t sure what it was rather than inject me with poison. To top it all off, they measured one of my cysts and referred to it as the ectopic pregnancy (even though they stated there was no sac or embryo present) in their report and dated my pregnancy at 5 weeks and 1 day. They never would have made this mistake if they had listened to me when I told them when my LMP was and had listened to me when I told them I ovulated on the 10th, so there was no medical way for me to be 5weeks 1 day prego.I don’t know what to do now, this has been the longest emotional rollercoaster of my life, and I’m tired, but I’m angry too. What would you do if this happened to you? I’m terrified, and simply trying to hold on at this point. I just really need some advice, if this has happend to anyone you know, were their children born normal? I can’t terminate this pregnancy if it h*I can’t terminate this pregnancy if it has made it this far, it would be our miracle baby. But I want someone to pay for their mistakes, just don’t know what to do.Methotrexate is a Class X drug that binds with folic acid to prevent cells from growing is what I learned through my own research, they never told me this. One story of a woman who had this same situation occur, her son died after 2.5 hours of living. I can’t take prenatal vitamins for a while until the drug gets out of my system, or it will severely damage my liver. I can’t believe they based their decision on an assumption. A “gray area”.
A: Your story is unbelievable…well, to a point. I think this is why I have formed such a distrust of most medical facilities. Anyway, that’s not a point I will go into. I agree with everyone else, I would talk to a lawyer about this. You trusted them and they really messed up. I truly hope to God that your baby is fine and will come out 100% healthy regardless of what they did to you. I can’t imagine going through that. You are a very strong woman to handle as much as you have! But do speak to a lawyer. No matter the outcome, rather it is ectopic or not, or rather your baby is born with defects or not, I believe you have a strong case. Not only have they put you and your child in danger, and not only have they put you and your husband through deep emotional distress, but it is flat out medical malpractice to give somebody a drug without telling them or providing information on the risks and side-effects of that drug. My prayers will be with you and your little one. And if you are pregnant please consider giving birth in a different hospital.
Did the surgeon mess up during the appendectomy and do we have a malpractice case?
Q: August 19, 2009 my husband 27 years old perfectly healthy called me and said he was home from work and having severe abdominal pain and that he was getting hot and cold and running a fever I was near the hospital at this time so I had his father pick him and bring him to the hospital which is about an hour and a half away from our home, but we’ve always heard great things about this hospital so we wanted him there. Also, I am going to this hospital for all my pre-natal care and also planned to deliver there. My husband walked into the emergency room slouched over and in agony. After we sit in the waiting room for about an hour they finally omit him to a bed in the ER. He lies in this bed in the ER for over an hour before recieving any sort of IV much less pain meds. EVENTUALLY approx. 3 hours later they have him drink the stuff for a CT and confirm that he has appendicitis. So, we’re glad to find out what the problem is and hoping to have it corrected. Well he goes into surgery and about 2 hours later the “Surgeon” comes out and says that he had a little problem removing the appendix that he ruptured it during removal and all the infection leaked into his body and that they had to flush it. So we’re glad they flushed it and they input a drain for infection to drain. So he makes us feel like everythings good to go. My husbad was admitted to a room and the entire time he was in the room he ran a fever, he was in pain for over 2 hours before he recieved any pain meds, he was spitting on his sleeve to clean his face because noone would bring him a wash rag, and then the doctor says okay your walking around and we’re gonna send you home with some pain meds and leave the drain in for five days. Well before five days could arrive we were BACK in the ER. My husband was in TERRIBLE PAIN, he could not breathe his stomach was bloated, and could not tolerate any food. And again, another CT was performed, he was bleeding internally from the appendectomy. They admit him to a room and do ANOTHER painful abdominal surgery by removing the blood from him abdomen and during this surgery inputting another JP drain. The doctor is very short and brief with us about this, we asked questions and he didn’t give us answers, he’s burning up with fever he can’t breathe, so we call our FAMILY PHYSICIAN to get involved, they make calls to immediately get a pulmonary specialist, and his surgeon wants to give him a MORPHINE drip for pain, he doesn’t care that he can’t breathe that his temperature is outrageous and that he feels like he’s going to die. Well the pulmonary specialist comes in immediately admits him to the ICU and orders tons of test, orders two pints of blood, and he’s making sure he’s getting great attention. Three days in ICU my husband improves so they admit him back down the “hell hole” as I would say, because I could write a book on HOW unsanitary those people are…anyways, two days later he’s released from the hospital STILL running a fever and can’t breathe very good. We discharged his surgeon, and got a new one in on the case, whose great and whose bed side manner is great, come to find out the surgeon he had has been there for 1 month. Now, 2 days at home he’s still running a 102-103 fever, and cannot breathe. He couldn’t lay down in bed because he couldn’t breathe.Back in ANOTHER ER a little farther down the road but MUCH better hospital they zip him right in the ER run a Chest XRay and a CT immediately. They had to put in a CHEST TUBE to drain the 3 liters of fluid off of his right lung (THIS IS NOT A FUN PROCEDURE) his right lung was collapsed and was only working off his left. The blood in his abdomen is still there puddling and growing some infection. Again he is admitted back in the hospital while on TONS of antibiotics and laying there with a chest tube. After 6 days they release him, putting him on a month of several antibiotics still with a JB drain in his side. He goes on the 21st of this month to MAYBE have it removed if the blood had drained completely. Keep in mind also, I’m sleeping in a chair 27 weeks pregnant scared to leave his side because he is not getting the care he needs. They were not changing the gauze they were not changing the IV’s every three days like the should have, they would not even rinse his pee jug out. I HAD TO DO IT!!!!!!!!!!!!!!!! He has lost an overall of 32 pounds. Is this something we could pursue to court or is this just one of those things where we got that end of the stick???
