What can I expect with starting Thyroid medication?

Q: After going around with my GP for at least 3yrs about not “Feeling Right” and getting the “your just depressed, here take this pill” of which i declined as I know I wasn’t depressed, I finally had my Thyroid checked when I became pregnant last May. I was told that my TSH was slightly elevated from what my OBGYN would like to see and I was producing Antibodies. Not that this is good news, but I was relieved and thought I would finally get some help. I was told to wait until about 6 mths post partum to re-test so that was not effecting the results. After requesting over the phone if I could get a referral to an Endocrinologist, as per my OB;s suggestion, and supplying my last test results my GP said she wanted to test herself. So she did, and found my TSH is now 15.1 – FreeT4 .08 – Free T3 (didn’t write down). All this was told to me over the phone by the nurse that stated the Dr. called in a prescription (synthroid) to start on right away and to come back in 2 mths to recheck blood work.That was it. No actual appt made with the Dr. to go over everything with me, drug choices, side effects to look for, course of plan we will try. Nothing. I was wondering if this is normal to just do over the phone? ( I have not see or talked to the Dr. for at least 6mths) Also, Are there any side effects I can expect initially or should look out for? How long to see any results? I do know that it depends on finding the right dosage to get optimal results, but am I going to get moody, weight gain, headaches……..I just want to be prepared. I have a near 7 mth old at home and still need to function (well at least as well as I have been up till now). I know I need it and will eventually feel better, I hope.Also, my Aunt has the same issue and told me she only truly felt 100% normal again when she went on Armour for a short while. Or at least a T3/T4 combo. Can I request this or do I have to put my time in on just Synthroid before I can even suggest that?? I am starting to search for a new GP as I just have never really liked this one and do not have much confidence in her. she is all about #’s and dismisses any time you try to say “how your feeling”….so frustrating.

A: Oh goodness, you are severely hypothyroid. Your doctor definitely should have explained things to you, but the standard course of treatment is Synthroid or its generic equivalent. Most people do not need Armour (T4/T3 combo) because they convert T4 to T3 just fine. Most likely they will start you on 25 or 50 mcg. Your doctor is spot on by checking you again in 8 weeks. TSH labs lag about 6 weeks behind, so that’s the reason for this. Since she didn’t explain these things to you very well, I will tell you to make sure you take your Synthroid in the morning and do not take it 2 hours after eating and 1 hour before eating. Also, do not take any vitamins containing calcium or iron for 4 hours before or after. All of these things interfere with absorption. I highly recommend you get with an endo, as a good one will look at the numbers but also assess by how you are feeling. You will need to get your TSH down around a 1 or below and your FT3 and FT4 in the upper 3rd of the reference range to feel better. Find an endo who is up to date with this. As far as side effects go, there aren’t too many unless you are overmedicated. You might feel a little hyperT after first starting the meds (trouble sleeping, fast heartrate, etc), but it usually only lasts for about a week while your body adjusts. Your TSH is so high that it probably would not happen to you anyway as you will most likely need to be on a higher dose to bring your TSH down to what it should be.

How many weeks pregnant and how long would it take for a blood test to come back to see if i was pregnant?
